				<div class="et_pb_text_inner">You wouldn’t be mistaken for assuming that trips and falls are only a common occurrence in just older people aged 65 and over. Even healthy people over 65 can have at least one fall a year and a simple fall can having serious consequences and complications. As we reach the 65+ age group, without regular strengthening exercises, our muscles can become weaker as we lead a more sedentary lifestyle. It is therefore, not a surprise that the risks are the same for Fibromyalgia and Chronic Fatigue Sufferers at any age.</div>

				<div class="et_pb_text_inner"><p>For a large percentage of Fibromyalgia and CFS sufferers, we lead a sedentary life style to try to prevent more pain and chronic fatigue. However, we are inadvertently causing more pain and other prominent symptoms. We soon find ourselves on the pain – fatigue – stress – sleep disturbance – stiffness &#8211; fog cycle. In our minds, exercise becomes unachievable.</p>
<p><strong> </strong>Sadly, with us unable to exercise the conventional way, our muscles decondition and become tight and painful and in turn makes us become more at risk of trips and falls, especially during flares and crashes when we are at our weakest.</p>
<p>From when Jane Gordon our treasurer and I qualified as Tai Chi for Health instructors 3 years ago, our priorities were to reduce the risk of trips and falls. There is also an added bonus because as we improve our strength and balance, our muscles and joints become less stiff and more flexible, helping to reduce pain.</p></div>

				<div class="et_pb_text_inner"><h3>Exercise is Tabu</h3>
<p>I have read on many forums and face book pages where sufferers are unable to exercise as it causes more pain or they are afraid to exercise as they will lose their benefits and be forced back to work. Just because we are choosing to do something about our health, it doesn’t mean we are cured or able to work, it means we respect ourselves enough to want to improve our symptoms so that we can have some quality of life, after all, we need our bodies for a few more years yet! If you keep reading comment after comment that you can’t exercise because it causes pain, you will start to believe it before you have even considered it. This adds to the ever-increasing obstacle of lack of motivation, confidence and understanding to overcome.</p>
<p>Yes, I agree that if you have not exercised for a long period of time and spend most of your hours per day on the sofa or in bed, there will be a degree of discomfort when you first start exercising and it will be foreign to your muscles. The key is to start at a very low intensity, practice regularly throughout the week and only increase the intensity when your body is ready. Compared to deconditioned pain, this has to be seen as positive pain as you slowly build your muscles up again. You have to have a degree of hope and determination to get through the early days.</p></div>

				<div class="et_pb_text_inner"><h3>Movement Therapy</h3>
<p>To ensure we don’t boom and bust, we also need to look at the type of exercise that we can do without exceeding our limitations. Identifying the type of movements, balance and strength exercises that will benefit us the most without causing extreme levels of PEM, Post Exertion Malaise and increase our pain levels?</p>
<p>Tai Chi and Qigong, Pilates, Yoga, swimming and gentle walks are all recommended as low impact exercises that help build healthy muscles and tone and strengthen our core muscles. Tai Chi especially offers all round benefits for both mind and body, helping to reduce stress. It is challenging to start with as we have to remember the moves and find some sort of coordination. Once you have completed a course it is very rewarding.</p>
<p>Like anything, practice improves performance and when I first started learning tai chi, I wondered why I was putting myself through the classes because I had very low energy levels, I couldn’t stand for more than a minute, I couldn’t hold my arms up and my fatigue levels made it difficult to follow. I am so glad I put myself through the first few weeks and it helped that I had my Fibro/ Tai Chi buddy, Jane to encourage me to keep going. The classes were so beneficial, it got me out of a mobility scooter and walking again and this was a key factor in bringing tai chi into the group programme and us both becoming Tai Chi for Health instructors with an extra Falls Prevention component.</p></div>

<p>From the time we qualified as instructors, we have developed a close partnership with Strictly no Falling (SNF), an Age UK Derby and Derbyshire project. SNF specialise in falls prevention. Our open community classes are registered under the SNF scheme and we have been very fortunate to receive not only a small set up grant to start the classes but during lockdown we received small grant payments to help us stay connected with our students and it paid for our Zoom instalments. SNF have a variety of Falls prevention classes including; chair-based exercises, posture stability, tai chi and Otago.</p></div>

<p>We had never heard of Otago until Jo Briggs the SNF coordinator spoke about it. Jo also mentioned that we would be able to recognise some tai chi movements in the programme and it would benefit our less able members. Jane and I have been training for the last two months and can’t wait to incorporate the exercises into our classes.</p></div>

				<div class="et_pb_text_inner"><h3>What is Otago?</h3>
<p>Otago is a home based strength and balance retraining programme, researched and developed by a team at Otago University in New Zealand. It has been proven to decrease the risks of falls by up to a third with people aged over 65 years. It has been designed so that participants can be prescribed individual exercises to be done conveniently throughout the day anywhere in the home. While you are waiting for the kettle to boil, you can fit in a cheeky one leg stand or heel walk to the sink.</p>
<p>The exercises are simple leg raises and steps which make you mindful about how you walk and stand. They progress from seated, fully supported to one hand support and no support. Further progression incorporates ankle weights in the strength exercises that start at 1Kg up to 5kg and increasing repetitions.</p>
<p>These exercises are perfect to help gradually build the strength and balance in our fibro legs. There is no set time so we can pace ourselves throughout the day and if you chose to do them all at once, it takes just 30 minutes. The balance exercises are also a great foundation for transitioning to the tai chi.</p>
<p>There is an initial assessment that will give both the instructor and the participant a foundation to work from. Exercises are prescribed to do 3 days a week and on the in between days, you are encouraged to go for a walk either for 30 minutes or broken into 3 x 10 minutes, depending on your fitness and capabilities.</p>
<p>Although Otago is designed for the home, we will be running classes so that everyone can come together for a bit of a social outing, especially after lock down. When they have reached the top level of Otago, they will have the option to progress to the tai chi for arthritis classes.</div>

<p>In between all this tai chi and Otago training, Jane and I have attended a Falls Ambassador training session. This has given us the chance to attend the Train-to-Train course, so we will be able to run falls awareness sessions for group members and students. We will also be looking at running a small falls prevention awareness event at our hall at some point in the future.<br />Having an all-round awareness of falls prevention is adding so much value and depth to our programme. We are able to help members and students of all ages and abilities. By tackling the potential issues early enough we will be able to help reduce the risk of trips and falls and prevent the consequences. Our aim is to build the confidence and capabilities of everyone at their own speed.</p></div>

<p>We use our hands for almost everything we do. With Fibromyalgia they let us down quite frequently. Symptoms include: –</p>
<p><strong>Difficulty gripping items such as pens, artist&#8217;s brushes and keys</strong></p>
<p>It feels like the strength has just disappeared. Writing becomes difficult and painful and the pen can just fall out of your hand. Losing sensation in your hands is especially annoying when you struggle with everyday tasks like unlocking the door, when your keys just keep falling out of your hands!</p>
<p><strong>Nerve related feelings all over the hands and into the fingers</strong></p>
<p>It feels like pins and needles almost all the time and it spreadsup your forearms. Typing even with the lightest pressure on the keyboard can send a cascade of pain from the point of contact up the digits, into the hands and up into the arms.</p>
<p><strong>Pain in the palms and the backs of the hands</strong></p>
<p>This can worsens during a flare.  The hands swell frequently, however this is none inflammatory.</p>
<p><strong>Tendon pain and Fascia Constriction symptoms</strong></p>
<p>This can be increased by repetition and changes in the weather.</p></div>

<p>Many sufferers try to reduce their pain levels by sitting. Sitting for extended periods every day weakens the muscles and increases pain. Trying to move causes so much pain and chronic stiffness that you tend to refrain from moving as much as possible.</p>
<p>I can’t express how important it is to keep moving.  Your hip flexor muscles are the only two muscles that connect the top of the body with the bottom. They have a really important job to do. They are the muscles that help you sit, stand and walk, if they tighten due to lack of movement it is highly likely you will need weeks of physiotherapy and muscle strengthening exercises just to get you mobile again. Tai Chi, gentle walks and swimming are recommended to help with mobility.</p></div>

				<div class="et_pb_text_inner"><h3><strong>Costochondritis &#8211; Fibromyalgia chest pain</strong></h3>
<p>This is a painful chest pain often associated with fibromyalgia.  It is an inflammation of the cartilage that joins the ribs to the chest bone, this inflammation is the cause of the sharp chest pains inside the chest wall.</p>
<p>This pain mimics the pain of cardiac problems including heart attacks and stroke.</p>
<p><strong>Symptoms include: &#8211;</strong></p>
<ul>
<li>Sharp stabbing pain in the front of the chest</li>
<li>Ribs that are sore to the touch</li>
<li>Pain on the left or right side of the chest</li>
<li>Upper chest pains</li>
<li>Burning pain in the ribs</li>
<li>Pain that radiates up the back of the neck and shoulders</li>
<li>Pain in your chest when you cough or sneeze</li>
<li>Pain that increases with activity, exertion or deep breathing</li>
<li>Pain that decreases with rest, movement or slow breathing</li>
</ul>
<p>&nbsp;</p>
<p>There are secondary symptoms of <strong>costochondritis</strong> which can include: &#8211;</p>
<ul>
<li>Rapid heart rate</li>
<li>Irregular heart rate</li>
<li>Shortness of breath or difficulty breathing</li>
</ul>
<p>&nbsp;</p>

Many of us when we were younger took on the challenge of stress and used it as a performance booster, working long hours, playing lots of sport, partying hard and all that. However, prolonged stress can have a negative effect on all of us and at some point, we burn out and hit the wall. Talking to many sufferers most previously had active lives and find it difficult to accept their limitations and try to fight them. The negative effects of stress can bring on pain and symptoms and with our thresholds to stress now very low, it doesn’t take a large amount to start a flare.</span></p>
<p><span style="font-weight: 400;">Back in the day of the cave man, there were not many things to think about apart from existing, catching food and avoiding being hunted. Man has a fight or flight receptor that kicks in the adrenalin to prepare to fight or run. So, man would make his choice and the energy source of the adrenalin would burn out. In today’s world prolonged stress and chronic pain is causing the flight or fight receptor to function 24/7 and not being able to do much in the way of burning off the adrenalin we end up in a vicious circle of pain – fatigue – weight gain. </span></div>

Many of us when we were younger took on the challenge of stress and used it as a performance booster, working long hours, playing lots of sport, partying hard and all that. However, prolonged stress can have a negative effect on all of us and at some point, we burn out and hit the wall. Talking to many sufferers most previously had active lives and find it difficult to accept their limitations and try to fight them. The negative effects of stress can bring on pain and symptoms and with our thresholds to stress now very low, it doesn’t take a large amount to start a flare.</span></p>
<p><span style="font-weight: 400;">Back in the day of the cave man, there were not many things to think about apart from existing, catching food and avoiding being hunted. Man has a fight or flight receptor that kicks in the adrenalin to prepare to fight or run. So, man would make his choice and the energy source of the adrenalin would burn out. In today’s world prolonged stress and chronic pain is causing the flight or fight receptor to function 24/7 and not being able to do much in the way of burning off the adrenalin we end up in a vicious circle of pain – fatigue – weight gain. </span></div>

				<div class="et_pb_text_inner"><p>When we first get diagnosed it is a huge relief because we are no longer crazy hypochondriacs to our Family and friends. Our expectations then, are that our GP is going to give us a miracle pain killer and laden us with heaps of information about the condition, we will go away feeling relieved and supported.</p>
<p>Sorry to disappoint, but as there is no cause or cure our GP’s can only prescribe what has been prescribed to thousands of us before and hope you get some temporary relief. They can only prescribe largely ineffective antidepressants and opiates, these drug us up and cause horrible side effects and with prolonged use, slowly rot our organs, leaving us in more pain and discomfort, leading to taking a larger cocktail of drugs to relieve the side effects. Suddenly, you are dependent on them with no support. We are then left isolated with our conditions because no one knows what to do with us.</p>
<p>At this point we become desperate and we will try anything to get some relief. Our condition has consumed us, we have lost our friends because we are either frequently cancelling our arrangements with them, or we are unable to keep up with the lifestyle we once had. Those of us with jobs are finding it difficult to recover for the next day and must make serious decisions about our capability and young parents get no respite at all. The continuous long-term stress exaggerates our symptoms and then begins to affect our mental health and relationships. In fact, someone worded it brilliantly the other day and said “it’s like living in a parallel world and you are watching everyone else going through life while time has stood still for you trapped in your body”. You are a victim, a sufferer, you are negative, you love to moan because it makes you feel better. But does it? And does it really have to be that way? <span style="color: #6b4591;"><strong>No, it doesn’t!</strong> </span>There is another way if you choose to take the challenge.</p>
<p>When the idea about starting the group first came about, I was in a very dark place. However, the only way I could see a support group for us to work was if we made it positive. Everything had to be positive, the group meetings, the Facebook page, the rules, the members, the future, everything.</p>
<p><span style="color: #6b4591;"><strong>I needed to become positive!</strong> </span>How did I get out of victim/ sufferer mode? Instead of looking at everything with dread and fear, I looked at it as putting pieces of a puzzle together, like research. I had to distance my negativity and almost get excited that I found a piece of the puzzle. Bear in mind that it is a huge puzzle!</p>
<p>First, I had to mourn the loss of my old self, this was really hard to do. I had 24 weeks of counselling that helped me through this process. This was not the easiest of processes as I had lost my identity and was consumed by Fibromyalgia so I had to look at reinventing myself on the things that I could do.</p>
<p>What could I do? What do I enjoy? What am I passionate about? Well I love helping people. I love to organise events. I loved exercise. These are all positive experiences and I just needed to adapt my experience and knowledge to become the new me. But how can I do this when I am in the biggest flare of my life.</p>
<p>Through the process of acceptance and loss I could identify my limitations. We are all different and our limitations can vary from day to day. I learned to listen to what my body was telling me and not to feel guilty if I needed to rest, no matter how long it took. I tried to stop boom and busting and paced myself instead, which is hard to do when you have responsibilities. It’s all about looking at your life and changing the stressful bits to help you. I could then plan what I needed to do and understanding that exceeding these would have consequences over the next few days. I knew if I had a busy day on a Monday that I would not have anything planned for the following 2 to 3 days just in case. It is all about forward planning and being organised.</p>
<p>Secondly, I had to lead by example and build my fitness up again. Now this is one of the hardest things to do for anyone, but then with a chronic illness as well most of us would just lock the door and go back to bed! Many sufferers especially if they are in ‘victim mode’ will deny any sort of exercise whether it be a gentle walk or a yoga class. This is the worst thing that you can do. The less you use the muscles in your body the more they waste away, the more pain you are in. The higher the risk of trips and falls. Many of us sit for many hours a day, which in turn puts more stress and pain on your movements when you need to get up out of your chair or bed. To add to the stigma of exercise many sufferers also think that they are going to lose their benefits because they are seen to be walking. This I can assure you is not the case. I have spoken to a manager at the DWP as I was concerned that sufferers are denying themselves vital care to maintain their symptoms. The DWP have reassured me that they do not have the funding to go spying on everyone and if gentle exercise is a key part of maintaining their symptoms then they should be encouraged to do it.</p>
<p>The hardest thing for me was that I had to realise that I couldn’t do the things I was doing before but my brain was geared to boxing training. Luckily, I found a brilliant personal trainer at the gym I go to who sat and listened to all my symptoms, concerns, issues about post training malaise etc, etc. He was so patient and wrote me a programme that was so basic, it was like a warm up in my previous life. I wouldn’t normally get out of bed for this. I wasn’t in the gym half an hour! After 3 months at this level I called the PT and arranged to improve my level and we did this a couple of times and very slowly my body adjusted. You see it’s like having an alien inside of you. You cannot wake the alien up otherwise it will punish you with pain and fatigue so you must tip toe so it doesn’t notice you and don’t wake it up. Sadly, I had a relapse and I ended up back at square one for over a year and it was even harder to start again. This is another acceptance that our day to day health is going to be like a roller coaster and we will hit rock bottom but the only way is up from there! I now try to swim twice a week and some days I can do 2 lengths and others a bit more. The goal is to walk through the doors and everything else is a bonus. I reward myself with a spa afterwards to warm the muscles to reduce the risk post swim malaise.</p>
<p>Thirdly, I had to reduce the intake of sugar as I was on self-destruct mode and piling in hundreds of calories of chocolate, biscuits and cake every day. I piled on 3 stone in a year and was dependant on my mobility scooter because of pain and I knew that my symptoms were exagerated by this but when you fall off the wagon nothing shocks you enough to get back on it again. It must be up to you to want it to achieve it. The way I have changed my eating habits is by learning how the gut works and what sugar does to you.</p>
<p>After listening to specialists, nutritionists and sufferers there is a way to improve our symptoms and it all starts in the gut! Yes, the gut! The saying you are what you eat is very poignant. We were made to eat meat, fish and vegetables, plus natural foraging. Today, we are continuously being encouraged to buy processed foods and confectionaries that have high contents of sugars and preservatives. But while this is making billions of pounds for the manufacturers no one is going to challenge this. Obesity rates are sky high along with diabetes and this will continue, as the foods on the shelves are cheaper than natural, clean foods. Some of our members are now on a high fat low carb diet and they are seeing some promising improvements with fatigue, fibro fog and energy.</p>
<p>Sugar is more <strong>addictive</strong> than heroin and causes inflammation which in turn causes pain and with the side effects of the pain killers being pain and weight gain, sufferers blame the side effects of the drugs when it is highly probable that they are helping it with a big slice of cake and a couple of bars of chocolate a day. On top of that you are feeding the bad bacteria in the gut which causes bloating and fatigue along with a list of other nasty conditions such as IBS. In addition, the long-term stress you are putting your body through increases the symptoms of Fibro Fog as your brain’s functionality is forced to reduce.</p>
<p>At the end of the day, if you didn’t take the ineffective drugs, there would be no side effects to blame for the sugar you consume which causes the inflammation, pain and fatigue. You would still be in pain but it would be more manageable without the long-term addiction of the drugs rotting your organs. You would be more active, keeping your muscles strong and flexible, reducing pain levels and the risk of trips and falls. Being more active improves your mental health and you will almost certainly be on the road to positivity, acceptance and self-management.</p>
